Enroll Your Children In The C.A.R.E. Program.

Complete the attached, C.A.R.E. Registration Form

  1. Fill out the form.
  2. Save completed form to your desktop.
  3. Attach it to an email addressed to rclocater@yahoo.com along with one image for each child you want enrolled.
Note:
  1. Images should be of individuals from the shoulders up and in front of a light colored background. Please do not send group images.
  2. Image needs to be in JPEG. Format and at least 640 X 480 pixel resolution or higher.   

We will reply by email confirming that we received your information and have included your children in the Care program for two years. 

ALL IMAGES ARE DELETED AFTER TWO YEARS
