Overview
Case
Management, Supportive Case Management and Intensive
Case Management Services are programs that provide
concrete services to Rockland County residents.
Residents must be 18 years and older and have a
functional disability that affects their daily living
resulting from serious and persistent mental illness.
Eligible residents are assisted through a process of
advocacy, monitoring, linkage, coordination and
integration of all services needed.
Some
of the services include applying for financial
entitlements, referral for supervised housing,
referral for vocational training, pre-employment
training and/or education, etc.
The
goal is to maintain community living and avoid
unnecessary psychiatric hospitalizations. Provision
of these services assures the continuity of care and
development of independent living skills to the
fullest extent possible.
All
the levels of Case Management are housed in Building F
of the Robert L. Yeager Health Center with the primary
services provided in the community.
General
Case Management
The
Case Management Program is comprised of case managers
and peer case managers and is under the direct
supervision of a Psychiatric Social Worker .
The
services are provided to individuals with a functional
disability due to serious mental illness.
Services are provided on a flexible basis, Monday
through Friday .

Supportive
Case Management
The Supportive Case Management Program is comprised
of Rockland County supportive case managers and
Mental Health Association supportive case managers.
Supervision is provided by a Psychiatric Social
Worker.
This level of case management services is provided
to individuals with a severe mental illness.
The individuals tend to have a higher risk and
higher need than those at the general case
management level.
Intensive
Case Management
The Intensive Case Management Program is a shared
staff, county & state, program.
The intensive case managers provide service to
individuals with the most serious functional
disabilities due to mental illness.
These individuals tend to have the highest risk and
have the highest need for intense services.
TRANSITIONAL
Case Management
The Transitional Management Program serves as a
liaison between the Criminal Justice System and the
Department of Mental Health.
The goal is to provide the services to incarcerated
consumers who are diagnosed with a mental illness.
