With today’s practice of shortened stays and
rehabilitation in acute care hospitals, our Summit Park Hospital is the answer.
It bridges the gap between hospital and home.
Summit Park is a 57-bed facility for individuals who no longer warrant staying
at an acute care hospital but who are still too ill to go home.
Individual Care Plans addressing medical care,
rehab and other services for each patient are created and reviewed every two
weeks. Patients benefit from our post-surgery, stroke and heart attack recovery
programs. Also provided are respiratory and wound care treatments, chronic
disease management and care guidance for concurrent medical conditions.
The length of stay averages between six and seven
weeks. Discharge Planning by RN’s begins within 48 hours of admittance. |
